About the position

The RN Care Coordinator at Aaron Manor Nursing & Rehab Center is responsible for planning and implementing nursing care for residents in a skilled nursing environment. This role ensures continuity of care between shifts, supervises CNAs and support staff, and advocates for residents' needs while maintaining a focus on quality care and compliance with regulations.

Responsibilities

• Document accurately on residents' conditions and required logs.
• Observe and report changes in residents' conditions to the medical team.
• Monitor and manage resident pain effectively.
• Administer medications safely and according to policy.
• Communicate effectively with the interdisciplinary team and document appropriately.
• Implement and ensure compliance with care plans established by the care team.
• Provide excellent customer service to residents and their families.
• Ensure comfort and care for residents at end of life or on hospice services.
• Maintain infection control protocols and use PPE appropriately.
• Report incidents and injuries to the supervisor promptly.
• Follow abuse reporting policies and procedures.
• Participate in skin care and pressure prevention protocols.
• Supervise and lead certified nursing assistants effectively.
• Demonstrate clinical competencies in nursing skills and procedures.
• Advocate for residents' rights and preferences.
• Manage medical orders and ensure timely transcriptions.
• Administer and document immunizations accurately.

Requirements

• Current Licensed Registered Nurse in State of New York.
• Graduate of an accredited Nursing program.
• Current CPR certification.
• Basic computer knowledge.
• Supervising experience in a nursing setting.
• Experience in wound care and care plan implementation.
