About The Position

The Coordinator role is responsible for overseeing the daily operations and ensuring the highest quality of care and compliance for residents in either the Burlington or Kenosha houses. This role requires excellent organizational, communication, and problem-solving skills. The Coordinator is required to serve as the 7-day on-call manager for the assigned houses every other week, which includes getting hours covered, managing call-ins, responding to emergency needs for residents, and supporting staff members.

Requirements

  • Must have and maintain a valid driver’s license in good standing.
  • Must have a minimum of a high school diploma or equivalent.
  • Must be able to give and receive feedback in a professional manner.
  • Must be proficient in reading, writing and speaking English.
  • Must be able to lift at least 20lbs.
  • Must be able to sit for extended periods in an office environment.
  • Must be able to traverse in home, office and community settings.

Responsibilities

  • Managing all aspects of resident medical care and needs, including appointments, documentation, and maintaining schedules.
  • Overseeing medical equipment care, ensuring Medical Administration Records (MARs) are up-to-date, and managing medication ordering and maintenance.
  • Ensuring compliance with state regulations, including monthly weights, fire and tornado drills, water temperature checks, equipment checks, and maintaining accurate resident documentation.
  • Updating Individualized Service Plans every six months and obtaining signatures.
  • Ensuring state survey compliance and accurate documentation, including incident report follow-up.
  • Managing house and resident needs, including cleaning supplies, grocery ordering, addressing general house needs, and ensuring resident financial needs are met.
  • Communicating general maintenance needs to the director.
  • Managing vehicle maintenance, including ensuring vehicles have gas and scheduling oil changes and other maintenance.
  • Organizing and communicating house activities and general postings, including planning outings and changing posted calendar boards.
  • Maintaining records for menu and activities.
  • Conducting in-house training for new caregivers.
  • Performing house pop-ins to check in with staff, evaluate resident care, and assess house cleanliness.
  • Reaching out for coverage of open shifts and scheduling needs.
  • Providing feedback, retraining, and written warnings to staff members.
  • Maintaining communication with the management team, guardians, and community care contacts.
  • Reading house logs and documentation.
