About The Position

Assists the manager and provides direct care in the operation of a Mental Health Supervised Apartment Program providing for the medical, physical, psychological and social needs and personal goals for individuals having severe and persistent mental illness on a 24/7 basis. Work is performed under general supervision and is guided by agency policies and procedures, quality assurance, public and private payor guidelines, professional practices and including ethical standards related to client interactions, state and federal licensure and accreditation regulations and guidelines.

Requirements

  • A bachelor's degree from an institution of higher education listed as accredited on the U.S. Department of Education College Accreditation database found on the U.S. Department of Education website or accredited by another accrediting agency recognized by the board
  • Evidence of completion of 80 hours of didactic education in a program recognized or approved by the board, unless such evidence was provided to the board to obtain a registration as a QMHP-trainee
  • Considerable experience in residential treatment programs for the mentally disabled including some supervisory experience.
  • Thorough knowledge of mental health disabilities and the principles and practices of residential treatment for individuals with serious mental illness, including assessment, treatment planning, behavior management, independent living skills training, supportive counseling, crisis intervention, discharge planning, and service coordination
  • Knowledge of applicable state and federal regulations, licensing and reimbursement requirements, resident rights, service plan development, staff training and evaluation, and liability considerations
  • Knowledge of community resources and support services available to residents, including eligibility, intake, and discharge processes
  • Working knowledge of case management practices, electronic health record systems, and residential facility operations, including maintenance, safety, security, and licensure standards
  • Basic knowledge of business and human resource management principles
  • Demonstrated ability to communicate effectively both orally and in writing
  • Establish and maintain positive working relationships

  • Ability to remain calm in crisis situations, interact professionally with residents and families, assess and document resident functioning, develop and implement service plans, and operate a motor vehicle.

Responsibilities

  • Orients, trains, schedules, supervises, and evaluates residential staff to ensure effective performance and compliance with program standards.
  • Provides direct care and instruction to residents in developing and maintaining daily living, social, and independent living skills.
  • Connects residents with appropriate community resources and assists with scheduling and attending appointments.
  • Transports residents to medical appointments, community activities, and other approved events.
  • Facilitates regular house meetings to promote communication, problem-solving, and resident engagement.
  • Supervises the self-administration of medications or administers medications as authorized; prepares and maintains all related documentation; monitors residents' medication management; and coordinates with staff, prescribing physicians, and pharmacies to ensure accurate and timely medication services.
  • Provides support across multiple mental health residential programs as needed, including assistance at alternate program sites to ensure continuity of care, adequate staffing, and compliance with program standards.
  • Retrieves medications from pharmacies and verifies accuracy and completeness of all medication orders.
  • Attends required job-related and program-specific trainings to maintain competency and compliance.
  • Participates in an on-call rotation to provide coverage during overnight, weekend, and holiday hours, as well as during the Program Manager's absence.
  • Documents resident progress and maintains all required charts, forms, reports, and related records in a timely and accurate manner.
  • Assists residents with personal financial transactions and related recordkeeping in accordance with program policies.
  • Prepares and submits purchase orders for program supplies and authorized expenses.
  • Coordinates and facilitates social, recreational, and therapeutic activities for residents.
  • Assesses resident needs and contributes to the development, implementation, monitoring, and quarterly review of Individual Service Plans (ISPs).
  • Completes and submits prior authorizations and appeals on behalf of the resident.
  • Conducts apartment inspections and emergency drills; inspects smoke detectors and fire extinguishers in accordance with policy; and maintains all required safety records.
  • Communicates and collaborates with family members, guardians, and other appropriate individuals involved in resident care.
  • Assists in maintaining the cleanliness, safety, and overall upkeep of program facilities, grounds, and vehicles.
  • Supports monitoring efforts to ensure compliance with applicable state and local licensing and regulatory requirements.
  • Provides supportive counseling, crisis intervention, and stabilization services to residents.
  • Enters service documentation and other required records into the electronic health record system accurately and promptly.
  • Performs other related duties as assigned.
