Responsibilities

  • Provides resident care and services under the direction of the Delegating Nurse and in accordance with physician orders, resident service plans, and facility policies and procedures, with emphasis on health monitoring, safety, comfort, dignity, and quality of life.
  • Assists residents with activities of daily living, including eating, feeding, hydration, grooming, bathing, oral hygiene, dressing, toileting, ambulation, transfers, positioning, and other personal care needs.
  • Observes residents for changes in condition, behavior, appetite, hydration, mobility, skin condition, mood, and overall functioning, and promptly reports concerns to the Delegating Nurse and appropriate supervisor.
  • Provides hands-on support to meet residents’ daily needs, recognizing that resident care includes comfort, cleanliness, nutrition, engagement, supervision, and overall well-being.
  • Assists with meal and dining services as needed, including meal set-up, tray service, feeding assistance, hydration rounds, snack service, dining room support, monitoring intake, and meal clean-up.
  • Assists with housekeeping and environmental support duties as needed, including making beds, changing linens, tidying resident rooms and common areas, handling laundry, cleaning resident-use items and equipment, disposing of trash, and helping maintain a neat, sanitary, and safe environment.
  • Assists with resident activities and engagement by escorting residents, encouraging participation, offering companionship, helping with activity set-up and clean-up, and supporting meaningful daily routines and social interaction.
  • Works collaboratively with nursing, dietary, housekeeping, and activities staff and is expected to step into support roles in any of these areas as needed to meet resident needs and maintain smooth community operations.
  • Acts as a liaison among residents, families, providers, pharmacy, and support departments to help plan for and respond to resident needs.
  • Documents care, services, observations, incidents, intake, and communication accurately, timely, and completely in accordance with facility standards.
  • Assists with admissions, transfers, discharges, room preparation, emergency response, evacuation procedures, training, drills, and other community support functions as assigned.
  • Promotes a positive work environment and demonstrates professionalism, teamwork, flexibility, accountability, and a resident-centered approach in all interactions.
  • Performs other duties as assigned to support resident care, regulatory compliance, and the overall operation of the community.
  • If holding a valid CMT certification and authorized to perform medication administration duties, follows all facility policies and procedures governing medication administration.
  • Accurately and safely prepares, administers, observes, and documents oral, topical, and other authorized medications in accordance with physician orders, Delegating Nurse direction, state regulations, and facility policy.
  • Observes and verifies that medications are ingested, applied, or otherwise received as directed, and promptly reports refusals, errors, side effects, adverse reactions, or concerns to the Delegating Nurse and appropriate supervisor.
  • Verifies that medications brought into the community by newly admitted residents are examined and identified in accordance with facility procedure by the attending physician and/or pharmacist, as applicable.
  • Demonstrates familiarity with common medical abbreviations, symbols, and terminology related to medication administration, treatment orders, documentation, and resident care.
