Responsibilities

  • Develop and maintain individualized care plans for clients based on their specific needs and preferences.
  • Coordinate services among caregivers, healthcare providers, and family members to ensure seamless communication and support.
  • Monitor client progress and adjust care plans as necessary to meet evolving needs.
  • Assist with medication administration and ensure proper documentation of medication schedules.
  • Conduct regular assessments of clients' physical and emotional well-being, including vital signs monitoring.
  • Provide training and support to caregivers on best practices in home community care, memory care, and assisted living environments.
  • Facilitate communication between clients and their families regarding care updates and concerns.
  • Advocate for clients' needs within the healthcare system to ensure they receive appropriate services.

Requirements

  • Proven experience in home care, assisted living, nursing home, or retirement home settings is preferred.
  • Familiarity with caregiving responsibilities, including lifting techniques for client mobility support.
  • Knowledge of medication administration protocols and vital signs assessment is highly desirable.
  • Strong organizational skills with the ability to manage multiple cases simultaneously.
  • Excellent interpersonal skills to build rapport with clients, families, and team members.
  • A compassionate approach to caregiving with a commitment to enhancing the quality of life for individuals in need.
