About the position

The Resident Care Coordinator is responsible for providing direct supervision of caregiving within the community, ensuring that residents are treated with respect and dignity while recognizing their individual needs and encouraging independence. The role fosters a homelike atmosphere throughout the community and involves various responsibilities including staff supervision, training, medication management, and communication with families regarding residents' needs and achievements.

Responsibilities

  • Provides direct supervision of resident care staff, monitoring job performance and collaborating with community leadership on evaluations.
  • Coordinates training and education of resident care staff on an ongoing basis.
  • Verifies associate hours on time cards and weekly hours report, completing nursing schedules to assure proper coverage.
  • Observes residents' health status, addressing concerns within scope of practice and reporting significant changes to the Health and Wellness Director.
  • Assists in medication management, ordering from pharmacy and maintaining records as per policies and procedures.
  • May administer medication to residents and review capabilities of those who self-administer medications.
  • Monitors staff in administration of medication, conducting frequent audits of medication administration records.
  • Records significant reactions of residents and notifies community leadership of conditions and reactions to treatments.
  • Communicates with families regarding residents' personal needs and achievements, reminding them of scheduled appointments.
  • Assists in organizing and developing service plan reviews with care team members and families.
  • Participates in the maintenance of resident charts and quality improvement activities.
  • Performs safety and infection control checks of the home and grounds.
  • Participates in community functions and shares on-call duties as required.

Requirements

  • Experience in a healthcare or senior living environment preferred.
  • Strong leadership and communication skills.
  • Ability to manage multiple tasks and prioritize effectively.
  • Knowledge of medication management and health monitoring.

Nice-to-haves

  • Experience with quality assurance and safety checks in a healthcare setting.
  • Familiarity with state regulations regarding medication assistance.
