About the position

The Remote MTM Pharmacist is responsible for delivering telephonic medication therapy management (MTM) services to improve member care. This role involves identifying medication-related problems and coordinating resolutions through effective communication with members, caregivers, and prescribers. The pharmacist will perform medication reconciliations with high-risk members and develop member-specific treatment plans based on recognized guidelines, utilizing skills in behavioral modification and interviewing to enhance patient health outcomes.

Responsibilities

• Deliver telephonic medication therapy management (MTM) services.
• Identify medication-related problems and coordinate resolutions.
• Perform medication reconciliations with high-risk members.
• Communicate medication-related issues to members, caregivers, and prescribers.
• Develop member-specific treatment plans based on treatment guidelines.

Requirements

• Bachelor's Degree in Pharmacy or PharmD from an accredited school of Pharmacy.
• Active pharmacist license in HI, TX, IL, FL, or PA.
• Strong organizational skills and attention to detail.
• Interpersonal skills for effective communication.

Nice-to-haves

• Previous pharmacy experience delivering MTM, specifically using the Outcomes MTM Connect platform.
• Basic computer skills and proficiency with Microsoft Office products.
• Experience maintaining focus and meeting goals in a work from home environment.
• Previous call center experience.
