About the position

The Rehabilitation Liaison PRN position at Our Lady of the Lake Rehabilitation Center involves evaluating potential rehabilitation candidates, serving as a liaison between the center and referral sources, and ensuring appropriate admissions based on medical necessity. The role requires collaboration with healthcare teams to develop treatment plans, conducting admissions committee meetings, and maintaining professional relationships with referral sources. The liaison also plays a key role in community education and marketing efforts, while ensuring timely insurance verifications and compliance with regulatory criteria.

Responsibilities

  • Evaluate potential rehab candidates referred to OLOL Rehabilitation Center and function as a liaison between OLOL and referral sources.
  • Conduct pre-admission assessments under CMS's IRF-PPS criteria to ensure appropriateness of admission and determine medical necessity.
  • Work with the healthcare team to develop rehab treatment/discharge plans.
  • Conduct weekly Admissions Committee meetings with the Medical Director to determine appropriate admissions and communicate information to the nursing team.
  • Maintain archives of denied patient assessments for the required period.
  • Develop, build, and maintain professional relationships with current or potential referral sources.
  • Contribute to community and professional education and assist with marketing efforts to ensure continuation and/or expansion of the referral base.
  • Obtain timely pre-certification and insurance verifications from third-party payors to support effective reimbursement for admissions.
  • Collaborate with HIM and physicians to ensure appropriate capture of coding that supports medical necessity criteria.
  • Communicate approval status with referral sources, patients, and families in a timely manner.
  • Access and utilize community resources and refer patients/families to additional support as needed.

Requirements

  • Three years of experience in general or specialty practice.
  • Associate's Degree in a relevant field.
  • Current Unrestricted Louisiana RN license (if nurse), Current Unrestricted Louisiana LPN license, or Current Louisiana Therapy license (if PT/OT/ST, certification also required if ST) or Licensed Physical Therapy Assistant.
