The Registered Nurse works as part of the care team and is responsible for providing outstanding patient care in the primary care/medical home model environment to provide care collaboration with clinicians, clinical support staff, behavioral health providers, community health workers, and external case managers in the design and provision of care as part of the approach to population health management through a cohesive, multidisciplinary care team. The Registered Nurse ensures that patients are provided with high quality care in a timely manner, receive prompt responses to phone call and patient portal inquiries, and remain up to date with evidenced-based preventative health guidelines while promoting wellness and disease prevention in the community. The Registered Nurse also identifies patients who may benefit from outreach efforts to promote health screening and related primary care services. During periods of wellness and injury, the Registered Nurse provides triage per clinical protocols and directs patient towards appropriate level of care in collaboration with the provider develops and understands patients' health care plan. The Registered Nurse functions under the direct supervision and authority of the Physician/Licensed Independent Practitioner when performing clinical tasks during patient care.
