About the position

The position involves upholding and supporting the philosophy, objectives, policies, and procedures of Community Health Systems, Inc. (CHSI). The role includes participating in quality improvement projects and being accountable for quality and Patient-Centered Medical Home (PCMH) initiatives. The individual will report to the Director of Nursing and the Clinic Administrator, seeking guidance on clinical protocols and operational duties. Responsibilities include facilitating quality improvement initiatives, conducting chart reviews, and ensuring accurate patient care documentation. The role also involves case management, patient advocacy, and collaboration with care teams to enhance healthcare delivery.

Responsibilities

  • Participate in quality improvement projects and activities.
  • Report to the Director of Nursing regarding clinical protocols and procedures.
  • Work with the Clinic Administrator on operational duties and process improvement.
  • Facilitate and implement quality improvement initiatives.
  • Identify, collect, and maintain information about the clinic's quality improvement activities.
  • Conduct monthly MA chart reviews related to workflow and quality programs.
  • Chart observations and data related to patient care.
  • Follow up with Providers and patients on critical abnormal lab results.
  • Assess, plan, direct, evaluate, and chart total nursing care.
  • Provide case management and act as a patient advocate.
  • Work with Referral Coordinators for appropriate case management.
  • Communicate challenges and opportunities across the care team.
  • Delegate care team members based on patient needs.
  • Assist Providers during examinations and procedures.
  • Provide Nursing Office Visits as necessary.
  • Oversee care team to ensure orders are followed.
  • Triage patients as needed.
  • Administer immunizations as needed.
  • Provide basic education to patients and families.
  • Ensure all clinical logs are completed accurately.
  • Handle and be knowledgeable of medical equipment.
  • Manage vaccines and medications including orders and tracking.
  • Ensure medical supplies are stored safely.
  • Follow clinic protocol for disposing of biohazardous materials.
  • Participate in training for best patient care.
  • Perform other duties as directed.

Requirements

  • Associate Degree in Nursing required.
  • Current California RN License required.
  • Current American Heart Association (AHA) BLS certification required.
  • Minimum of two (2) years of nursing experience preferred.

Nice-to-haves

  • Bachelor's Degree in Nursing (BSN) preferred.
  • Previous supervisory experience preferred.
