About the position
The Registered Nurse (RN) Case Manager at Angels Care Home Health is responsible for coordinating all aspects of patient home care. This role involves interacting with various medical disciplines, providing professional care to patients, conducting admission visits, determining eligibility for home care services, and developing individualized care plans.
Responsibilities
- Coordinate all aspects of the patient's home care.
- Interact with other medical disciplines as needed.
- Provide professional care to the patient.
- Perform the admission visit to the patient in the home.
- Determine the patient's eligibility for home care services.
- Develop the plan of care to be followed.
Requirements
- Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
- Current state license as a Registered Nurse.
- Current state Driver's License.
- One year's experience as a Registered Nurse, two years preferred.
- Proof of current CPR and Hepatitis consent/declination.
- Reliable transportation with valid and current auto liability insurance.
- Must pass State regulated Criminal History Background check.
