About the position

The Registered Nurse (RN) at One Medical plays a crucial role in delivering comprehensive and coordinated care to adults aged 65 and older within a primary care setting. This position emphasizes patient education, clinical nursing care, and collaboration with a multidisciplinary team to enhance patient outcomes and ensure a high-quality healthcare experience.

Responsibilities

• Provide direct patient assessment and care for adults 65 and older
• Conduct phone and walk-in triage for patients
• Oversee anti-coagulation clinic activities and patient monitoring
• Act as a transitions of care coordinator for post-discharge visits and calls
• Participate in the clinical development of Health Coaches
• Engage in process and quality improvement initiatives
• Perform IV initiation, drug therapy management, and phlebotomy as needed
• Administer IM/SQ/PO medications and vaccines
• Provide treatments and assist providers with procedures
• Perform diagnostic tests including EKG, ABI, and Spirometry
• Oversee in-house laboratory processing
• Conduct point of care testing such as Hemoglobin A1c and INR
• Provide patient teaching, education, and support
• Participate as an active care team member across multiple disciplines
• Perform wound care

Requirements

• Active RN license in WA/Compact RN license
• 2+ years of nursing experience
• Excellent verbal and written communication skills
• Strong interpersonal and teamwork skills
• Critical thinking and judgment in clinical care
• Compassionate and open-minded approach to patient care
