Requirements

  • Related Training and Mass RN License.
  • One (1) year experience in a community health setting.
  • Professional appearance and demeanor.
  • Excellent Writing Skills.
  • Possession of a valid Driver’s license, a good driving record and a willingness to use agency vehicle in the course of employment.

Nice To Haves

  • Experience with adults with developmental disabilities.
  • Supervision and training experience.
  • Day Habilitation and/or medical residence supervisory experience highly desirable.
  • Knowledge of MAP training, and ability to re-certify and train staff highly desirable.

Responsibilities

  • Oversee health and safety supports for all individuals.
  • Complete all necessary medical documentation.
  • Monitor/Oversee health and safety services for individuals in the Day Habilitation Program, while they are in the center or community, consistent with Medicaid, DDS, DPH and OSHA safety/ health standards, human rights, dignity and choice.
  • Work with the Nursing Team to maintain agency health standards and medication administration.
  • Provide supervision to Nursing Team and direct care staff at Price Center medical residence.
  • Assure highest quality care and ensuring adherence to funder regulations, CARF, and DPH regulations.
  • Provide emergency medical treatment to individuals, as needed, and routine medical treatment as prescribed by personal physicians, and document the same.
  • Provide healthcare services for all program recipients.
  • Upkeep of all nursing, medication, and health records in individual confidential files; complete and submit state required medication administration and storage reports.
  • Complete Agency accident/injury forms.
  • Review all new referrals and manage medical admission requirements.
  • Perform assessments for the individuals to determine medical needs and make recommendations for intervention.
  • Act as primary contact with residential nursing staff and primary physicians, obtaining required reports/approval.
  • Assess and make recommendations for equipment to support individual’s safety and comfort.
  • Participate in the Interdisciplinary Team's continuing individual evaluations, documenting recommendations and advising staff of any medical issues.
  • Conduct individual and/or group employee trainings for agency safety plan procedures, universal precautions, health/ safety standards, and individual health needs.
  • Participate in the quarterly inspections done by the Safety Committee and document any findings or recommendations.
  • Maintain a professional appearance and demeanor, treating all individuals, staff, and families as valued adults.
  • Work with individuals and small groups, per ISPs, to develop independent living, health, hygiene and safety skills.
  • Provide supervision for L.P.Ns on staff.
  • Support program participants’ right to express their religious affiliations and cultural traditions through preferred and relevant activities.
  • Support diversity of individuals served through respect of religious preferences and/or cultural backgrounds.
