Responsibilities

• Coordinate care for terminally ill patients in various settings.
• Provide comprehensive medical care and support to patients and their families.
• Document all care provided and administer medications as required.
• Communicate patient status and changes to the interdisciplinary team.
• Act as a liaison to other caregivers and spiritual care providers.
• Deliver critical hands-on care to patients.

Requirements

• Must be a graduate from an accredited school of nursing.
• Current professional license as a Registered Nurse in New York State.
• Minimum of one year of work experience as a professional nurse within the past five years in relevant settings.
• Strong clinical and communication skills are essential.
• Valid driver's license and reliable transportation required for travel.

Nice-to-haves
