About the position

The position functions as part of the Home Therapies dialysis healthcare team, ensuring quality home patient care in accordance with company policies and procedures. The role supports the Quality Enhancement Program (QEP) and Continuous Quality Improvement (CQI) activities, focusing on patient satisfaction and process improvement to meet quality goals.

Responsibilities

  • Provides safe and effective training and oversight of home dialysis therapies to patients.
  • Participates in staff training and orientation of new staff as assigned.
  • Submits CQI information to Clinical Manager for completion of HT QAI Template.
  • Assists in the identification and evaluation of candidates for the Home Therapies Program.
  • Trains home patients and their families on the operation and maintenance of home therapies equipment.
  • Educates patients and families regarding End Stage Renal Disease (ESRD) and the Kidney Options Program.
  • Assesses patient and family readiness for home dialysis treatments.
  • Provides ongoing education to patients regarding their renal dialysis and related health conditions.
  • Delivers safe and effective home therapy support care to patients with ESRD.
  • Communicates patient-related issues to the physician as needed.
  • Reports adverse patient events and equipment problems to management and physicians.
  • Coordinates the transfer of patients to the hemodialysis clinic when necessary.
  • Reviews, transcribes, and enters physician lab orders accurately.
  • Maintains competency with emergency operational procedures and initiates CPR when necessary.
  • Administers medications as prescribed and maintains records on controlled substances.
  • Maintains skill level in the operation of home dialysis therapy-related equipment.
  • Documents all relevant patient data and treatment parameters appropriately.

Requirements

  • Graduate of an accredited School of Nursing (R.N.)
  • Current appropriate state licensure
  • Minimum of one year medical-surgical nursing experience preferred
  • Minimum of one year dialysis experience preferred
  • Competent at operating all Home Therapies related equipment
  • Successfully complete CPR certification with maintenance as required
  • Demonstrated teaching abilities

Nice-to-haves

  • ICD-9 coding training
  • ICU experience preferred
  • Peritoneal experience preferred
