About The Position

Angels of Care Home Health is looking for Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide direct, one-on-one medical care in the New River, AZ and surrounding areas. The role involves caring for infants, children, and/or adolescents in their homes according to their active plan of care. The agency has been serving the medically fragile community nationwide since 2000 and is founded on values of Heart, Advocacy, Love, Outreach, and Speed.

Requirements

• Active RN or LPN/LVN license
• Provide care in a client home setting

Responsibilities

• Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
• Conduct on-going patient care and assessments.
• Administration of prescribed medication, treatments, and therapies.
• Coordination of care
• Educate family members on patient clinical care to enhance positive outcomes
• Preventative initiatives to protect quality of care for patient
