About the position

The Registered Nurse (RN) position at Care Hospice involves planning, organizing, and directing hospice care for patients in their homes and communities. The RN will conduct comprehensive assessments, develop care plans, and provide professional nursing care with a focus on palliative actions. This part-time role requires collaboration with patients, families, and interdisciplinary teams to ensure quality care and support for individuals facing end-of-life challenges.

Responsibilities

  • Completes initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs.
  • Provides professional nursing care utilizing all elements of the nursing process.
  • Assesses and evaluates patient's status by writing and initiating a plan of care and regularly re-evaluating needs.
  • Develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
  • Initiates appropriate preventive and rehabilitative nursing procedures, administering medications and treatments as prescribed.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Assists the patient with activities of daily living and facilitates efforts toward self-sufficiency and comfort care.
  • Acts as Case Manager when assigned, coordinating patient care for assigned caseload.
  • Completes, maintains, and submits accurate clinical notes regarding patient's condition and care given.
  • Communicates with the physician regarding the patient's needs and reports changes in condition.
  • Teaches the patient and family/caregiver self-care techniques and provides medication and diet instructions.
  • Maintains a safe environment for the patient and assists in providing continuity of care.
  • Participates in on-call duties as defined by the on-call policy.
  • Supervises ancillary personnel and delegates responsibilities when required.

Requirements

  • Registered nurse with current licensure to practice professional nursing in the state.
  • Graduate of National League for Nursing accredited school of nursing.
  • Maintains a current CPR certification.
  • Must be a licensed driver with an insured automobile in good working order.
  • Minimum of one year of experience as a professional nurse within the last three years or a baccalaureate degree in nursing with a current RN license.
  • Self-directed and able to work with minimal supervision.
  • Demonstrates excellent observation, problem solving, verbal and written communication skills.

Nice-to-haves

  • Management experience (not required)
  • Experience in hospice care (preferred)
  • Ability to communicate in languages other than English (preferred)
