We provide care at home or in settings such as assisted living, a nursing home, or the hospital. Our offices are located in Sunnyvale, South San Francisco, and Oakland. Patients and their families know us for our personalized, high-quality care, delivered with empathy, kindness, and respect. The Registered Nurse - Home Health position is based in Sunnyvale, covering the Sunnyvale - Gilroy patient territory. This is a full-time role with a schedule of Monday to Friday from 8:30 am to 5 pm, including rotating weekends. A sign-on bonus of $9,000 is available (conditions apply). The compensation range for this position is $56.95 - $78.30 per hour, which may vary based on circumstances at the time of posting.

Responsibilities

  • Perform an initial, comprehensive assessment.
  • Document observations, clinical findings, problems, skilled interventions, goals and discharge plans.
  • In consultation with the assigned Clinical Team Manager, initiate and regularly re-evaluate and revise the plan of care.
  • Assess the need for the services of other team members (PT, OT, ST, MSW, and HHA).
  • Provide and document skilled care on all visits (includes skilled observation of the patient's condition, skilled care, and procedures and teaching of the patient and/or family).
  • Obtain and document physician orders.
  • Perform Home Health Aide supervisory visits per regulatory requirements.
  • Coordinate care with the assigned Clinical Team Manager, physician and other members of the home care team, informing them of significant changes in the patient's condition and needs. Document these communications.
  • Follow established standards for point of service technology, documentation, and synchronization.
  • Submit weekly visit schedule of assigned patients. Collaborate with Clinical Team Manager to address scheduling needs.
  • Perform re-certifications, resumption of care, transfers, and discharges as requested by the assigned Clinical Team Manager. Complete and submit all related documentation.
  • Attend and actively participate in the clinical team multidisciplinary patient conference.
  • Demonstrate established clinical competencies.
  • Participate in agency sponsored in-service education.
  • Participate in quality improvement activities.
  • Assist in development of agency protocols, procedures and policies as requested.
  • Assess, develop, organize and deliver teaching materials for assigned home care patients as appropriate.

Requirements

  • Current California RN license.
  • Current CPR Certificate.
  • Minimum of one year of nursing experience required.
  • Current California driver's license and automobile insurance.
