About the position

Hospital Sisters Health System (HSHS) is seeking a Registered Nurse-Home Health to join our team. The Registered Nurse-Home Health, in conjunction with the Case Manager, utilizes physicians' orders and professional skills to develop and implement a home health plan that meets each patient's specific needs and is in compliance with accrediting organization standards, federal and state regulations, reimbursement guidelines, as well as agency policies and procedures. The Registered Nurse-Home Health is responsible for planning, coordinating and documenting care for an assigned caseload of patients. Participates in Quality Assessment and Performance Improvement (QAPI) activities.

Responsibilities

  • Utilize physicians' orders and professional skills to develop and implement a home health plan.
  • Ensure compliance with accrediting organization standards, federal and state regulations, and agency policies.
  • Plan, coordinate, and document care for an assigned caseload of patients.
  • Participate in Quality Assessment and Performance Improvement (QAPI) activities.

Requirements

  • Graduate from an accredited and approved school of nursing accepted for licensure in the state of practice.
  • Licensed as a Registered Nurse (RN) in the state of practice.
  • Valid driver's license, automobile insurance, and own transportation for home visits.
  • Basic Life Support (BLS) certification is required.

Nice-to-haves

  • Bachelor's Degree in nursing.
  • Prior experience in home health or hospice.
  • 1 year acute nursing experience.
