About the position

The position involves performing an age-specific plan of care for a designated group of patients using the nursing process, which includes assessment, diagnosis, outcome identification, planning, implementation, and evaluation of patient care. The role requires collaboration with physicians and other health team members to coordinate and implement procedures and treatments. Leadership skills and clinical judgment are essential for coordinating patient care and directing the activities of the patient care unit team.

Responsibilities

  • Conducts patient interviews and explains policies and procedures to patients and significant others.
  • Assesses gastrointestinal, cardiovascular, respiratory, renal, and neurological health status; determines mobility, sensory deficits, prostheses use, and skin condition.
  • Assesses level of pain and pain management; communicates and documents patient's physiologic health status and plan of care.
  • Assesses patient's psychosocial health status and elicits perception of medical/nursing care and expectations of care.
  • Identifies coping mechanisms, knowledge level, and ability to comprehend; identifies cultural/ethnic requirements to reduce anxiety and ensure patient satisfaction.
  • Develops patient outcome statements and establishes individualized patient goals; identifies care activities and establishes priorities necessary to achieve expected outcomes.
  • Coordinates the cost-effective use of supplies, equipment, and medication to achieve expected outcomes; documents the plan of care and collaborates with health team members.
  • Maintains constant vigilance over patient care to ensure safety precautions are followed.
  • Exercises professional skills related to the plan of care; reassesses patients as needed and revises the plan of care appropriately.
  • Correctly administers prescribed treatments and uses equipment necessary for patient care; provides emotional support to patients and significant others.
  • Keeps accurate documentation of patient's treatment, activity, and condition, as well as responses to medical and nursing interventions.
  • Acts rapidly and effectively during emergency situations, managing self, patients, and other employees; provides a calm atmosphere.
  • Participates in planning for discharge and coordinates referrals; provides discharge instructions to patients and significant others.
  • Evaluates care provided for patient outcomes and measures effectiveness of care; performs variance analysis related to outcome data for performance improvement.
  • Collaborates with care team members in planning and carrying out treatment regimens; provides direction to other members of the care team.
  • Accurately interprets and implements treatment regimens as prescribed by the physician; assists during treatments and/or diagnostic procedures.
  • Educates patients and significant others related to drug and food interactions; participates in patient and family education.

Requirements

  • Graduate from an accredited School of Nursing.
  • Bachelor's Degree in Nursing preferred; must be enrolled in an accredited BSN program within two years and obtain a BSN Degree within five years of job entry date.
  • Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
  • 2+ years of critical care RN experience preferred.
