About the position
The Registered Nurse (RN) - Case Manager is responsible for coordinating patient care and ensuring that individuals receive the necessary services and resources for optimal health outcomes. This role involves collaboration with healthcare professionals, patients, and families to create and implement personalized care plans, monitor patient progress, and facilitate continuity of care.
Responsibilities
- Conduct comprehensive assessments of patients' medical, psychological, and social needs
- Develop and implement care plans in collaboration with the healthcare team
- Coordinate services and resources to ensure patients receive appropriate care
- Educate patients and their families on health management and preventive care
- Monitor patients' progress and make adjustments to care plans as needed
- Communicate with healthcare providers and community resources to facilitate continuity of care
- Document patient interactions and care plans in a timely and accurate manner
Requirements
- Valid RN license
- Bachelor's degree in Nursing (BSN) preferred
- Previous experience in case management or related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Knowledge of healthcare laws and regulations
- Basic computer skills
