About The Position

Color Health is revolutionizing cancer care with the nation’s first Virtual Cancer Clinic, delivering high-quality, physician-led multidisciplinary care across all 50 states. Our innovative, guideline-based approach spans cancer screening, risk assessment, prevention, diagnosis, treatment support, and survivorship. In addition to personalized direct medical care, our services include cancer genetics risk assessment, nutrition, mental health support, and at-home cancer screening diagnostics. Using technology-driven, patient-centric solutions, Color is transforming how employers, unions, health plans, and governments address cancer. Color’s goal is to close critical gaps in cancer care, improve cancer outcomes, and guide patients with empathy through their healthcare journeys. In June 2023, Color and the American Cancer Society (ACS) introduced a cancer screening and prevention program for employers and labor organizations, combining the ACS’s expertise in cancer screening, prevention, and risk reduction with Color’s leadership in population-scale healthcare delivery. Since announcing our partnership with the American Cancer Society, our program has expanded to support diagnosis management, cancer treatment and care, and survivorship. Through our Virtual Cancer Clinic, Color is helping employees and members diagnose cancer at an earlier, more treatable stage; caring for patients going through their diagnostic journeys with empathy and efficiency; helping cancer survivors navigate return-to-work challenges and complex clinical needs; and closing critical gaps in cancer screening for high-burden cancers. We are seeking a dedicated and experienced Registered Nurse (RN) to join our Virtual Cancer Clinic care team. This role is critical in supporting patients who report symptoms, need education and follow-up after abnormal screening results, and require timely coordination to get the care they need. You work in partnership with our care team including physicians, advanced practice providers, genetic counselors and care advocates. The ideal candidate will be passionate about cancer screening & prevention, adept at using telehealth technology, and committed to improving outcomes. You deeply understand the challenges that patients face, the importance of cancer screening, and you are passionate about helping large populations achieve better health outcomes.

Requirements

  • Bachelor of Science in Nursing (BSN) required.
  • Unrestricted active Registered Nurse (RN) license with eligibility to obtain additional state licenses as needed.
  • Minimum of 7 years of nursing experience, including at least 5 years in primary care with result follow-up experience.
  • Outpatient experience required.
  • Related certifications, such as Ambulatory Care Nursing Certification (AMB-C), Certified Case Manager (CCM), Nursing Case Management Certification (CMGT-BC™) or oncology-related certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) required or to be obtained within 12 months of hire.

  • Excellent organizational skills with the ability to multitask and thrive in fast-paced environments.
  • Strong communication skills, both verbal and written, with a focus on empathy and active listening.
  • Comfort with technology, including video chatting, electronic health records, and other telehealth tools.
  • Regular availability during Pacific Time business hours.

Nice To Haves

  • Active Nurse Licensure Compact (NLC) multi-state license is preferred.
  • Inpatient and/or oncology experience are pluses.
  • Professional fluency in English and Spanish, including the ability to communicate using medical Spanish, strongly preferred.
  • Multi-state Nurse Licensure Compact (NLC) license.

Responsibilities

  • Serve as a vital clinical resource for our cancer screening & prevention population by assisting with triage, care coordination, and education via phone & async messaging.
  • Conduct clinical and needs assessments to identify and address patient needs.
  • Perform telephone visits with patients who report symptoms, ensuring these symptoms are being addressed appropriately, helping coordinate follow up care, and escalating to the next level of care when needed.
  • Follow up on abnormal cancer screening results by ensuring patients receive results, educating patients on next steps, helping coordinate follow up appointments until the appropriate work up is complete.
  • Review annual patient engagement surveys to verify if screening needs have changed or patients have new clinical needs to be addressed.
  • Provide compassionate, empathetic care, establishing close, trust-based relationships with patients, and going above and beyond to help and support the patient.
  • Collaborate with Care Advocates to address non-clinical support and care needs.
  • Develop clinical protocols and best practices to coordinate care and provide clinical support to patients from screening to survivorship, in close collaboration with other Color Care Team members (e.g., Color Medical physicians and NPs, Care Advocates, Genetic Counselors).
  • Actively engage with patients to address their clinical needs or issues, such as symptom management, care escalations, provider referrals, and planning for in-person care appointments.
  • Participate in care team sessions to monitor member progress, provide updates, and collaborate on targeted support plans.
  • Maintain thorough and detailed documentation of encounters with patients.
