About The Position

Dignity Health East Valley – Gilbert, operated jointly with Lifepoint Health and Dignity Health, is seeking a Registered Nurse (RN) for their Acute Rehabilitation unit. This full-time night position involves a 3x12 schedule, working nights from 6 PM to 6 AM, with rotating weekends. The role emphasizes providing superior patient care, prioritizing patient well-being, and making a significant difference in patients' recovery journeys. Requirements

Current state RN license.
Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing, or Nursing Diploma program.
Minimum six months of Medical/Surgical experience in an acute care setting preferred.
Current state licensure as a Registered Nurse.
CPR/BCLS certification required.

Nice To Haves

ACLS & CRRN preferred.

Responsibilities

Monitors and manages patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance.
Consults with nursing staff and multidisciplinary team regularly to evaluate patient's status and appropriateness of medical care, including admission, length of stay, transfer and discharge.
Monitors patient and family satisfaction.
Responds to questions and complaints from patients, family members, and payors regarding care.
Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources.
Provides discharge education and resource referrals to patients.
Performs chart review to identify actual or potential issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.
