About the position

This position will be allocated to our Chatsworth location with future plans to transition to our Thousand Oaks location late summer / early fall of 2024. Momentum is a leading provider of care services in Intermediate Care Facilities (ICF) for individuals with disabilities. Responsibilities

  • Provide skilled nursing care to individuals with disabilities in an Intermediate Care Facility setting
  • Implement and oversee the health care plans for residents
  • Administer medications and perform medical procedures, as needed
  • Collaborate with a multi-disciplinary team to ensure the well-being of residents
  • Maintain accurate medical records and documentation
  • Offer support and guidance to: License Vocational Nurse (LVN), Direct Support Professionals (DSP)
  • Participate in complex case management and engage in continuous learning opportunities

Requirements

  • Valid Registered Nurse (RN) license in the state of California
  • One (1) year experience as an RN; required
  • CPR certification
  • Strong communication and interpersonal skills
  • A genuine passion for providing quality care and support to individuals with disabilities
  • Ability to work effectively in a team-oriented environment
  • Willingness to adapt to the needs of the residents and facility
