About the position

The position involves applying the nursing process to plan and implement patient care, including education and continuum of care planning. The role requires effective communication with the healthcare team and collaboration in establishing patient goals, ensuring effective outcomes, and engaging in professional growth activities.

Responsibilities

  • Collaborates in establishing the plan of care, coordinates and implements care delivery while anticipating patient care needs across the continuum.
  • Analyzes comprehensive information pertinent to the patient's care and management to determine nursing diagnosis.
  • Continually utilizes the nursing process to evaluate patient's progress towards goals and applies appropriate interventions to ensure effective outcomes.
  • Employs strategies to promote health and a safe and healthy environment.
  • Additional duties may be assigned as needed.

Requirements

  • LIC-Registered Nurse - Licensure-Others
  • Relevant Work Experience
  • Graduate of an approved discipline specific program

Nice-to-haves

  • Experience in gathering pertinent clinical data that aid in referral, treatment, or other primary care pathways.
  • Ability to adapt to changes in patient care and area needs with minimal disruption.
