Responsibilities

• Conduct detailed clinical assessments of patients.
• Prioritize patient and family needs in care planning.
• Create, implement, and evaluate individualized care plans.
• Adjust care plans based on patient responses to achieve desired outcomes.
• Build therapeutic relationships with patients and families.
• Educate patients, families, and community members about healthcare choices.
• Assess and document learning needs of patients regularly.
• Collaborate with colleagues and community to develop care plans.
• Delegate tasks to team members and follow up on their completion.
• Utilize problem-solving strategies to address patient and family concerns.

Requirements

• Graduate of an accredited nursing program.
• Current and valid nursing license.
• BLS certification or ability to obtain within 30 days of starting the role.
• Deep understanding of nursing and patient care standards and procedures.
• Knowledge of nursing laws, rules, standards, and guidelines.
• Proficiency in medical terminology and disease management.
• Respect for medical ethics and patient privacy rights.
• Exceptional verbal and written communication skills.
• Positive social skills for effective interaction with diverse groups.
• Strong problem-solving and critical thinking abilities.
• Basic computer skills, including Microsoft Office and electronic medical documentation.
