About The Position

The Department of Mental Health (DMH) is seeking a Registered Nurse for the Adult Community Clinical Services (ACCS) Integrated Team. This role involves providing behavioral and physical health monitoring, oversight of the Medication Administration Program (MAP), completing Physical Health Assessments (PHAs), participating in the development of Individualized Action Plans (IAPs), and delivering trauma-informed, person-centered community-based interventions. The RN will target skill development in Health & Wellness, conduct routine assessments, provide medical care, and collaborate with care coordination entities to ensure access to appropriate medical services. The selected candidate will function as the ACCS Team RN, offering nursing oversight, consultation, and problem-solving to LPNs and MAP Certified team members.

Requirements

  • Knowledge of the principles and practices of nursing.
  • Knowledge of the terminology and standard abbreviations used in nursing.
  • Knowledge of the problems in providing nursing services to special client groups such as the mentally, physically, and/or emotionally handicapped.
  • Knowledge of standard nursing policies, procedures, methods and techniques.
  • Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing.
  • Recent graduates from an approved school of nursing must have applied for the first available registered nurse examination.
  • One year of full-time, or equivalent part-time, experience as a registered nurse in a recognized hospital, clinic or medical facility, or any equivalent combination of the required experience and substitutions.
  • Possession of a current and valid Massachusetts Motor Vehicle Operator's license may be required.

Nice To Haves

  • Experience working in a 24/7 residential program and community setting for adults with mental illness.

Responsibilities

  • Ensures meeting of MAP standards through education, coordination, oversight implementation, and QA reviews of the MAP process.
  • Provides initial MAP testing to new hires/transfers, recertification training and testing for existing staff, and conducts regular MAP inspections of all DPH registered sites.
  • Serves on an ACCS Integrated team by attending meetings, presenting psychosocial history, mental status, diagnosis, treatment provided, and planned treatment for assigned cases, and participating in case discussions.
  • Assists in the development, understanding, and implementation of IAPs for assigned caseloads, involving persons served and significant others.
  • Assesses the ongoing needs of assigned persons served through supportive counseling, record review, home visits, and regular contact with persons served and families.
  • Functions as an advocate and liaison for persons served in interactions with primary and other health care providers, conferring on medical and medication issues.
  • Provides rehabilitative interventions to assist persons served in building skills, accessing resources, managing symptoms, and living productive lives, considering SAMHSA’s 8 Dimensions of Wellness.
  • Reads and communicates pertinent information, attends daily rounds and staff meetings, and informs supervisor of critical information.
  • Transports or arranges transportation for persons served as needed.
  • Assesses persons served mental status and assists with coping strategies, natural supports, and safety planning.
  • Maintains person served records in a timely manner by documenting in EMR and paper records, and completing all other related forms.
  • Provides primary nursing care by assessing health status, recording health data, evaluating outcomes, and adjusting care with other service providers.
  • Communicates pertinent information to ACCS staff, other treatment providers, and care coordination entities.
  • Transcribes and ensures implementation of physician’s orders.
  • Conducts self-medication readiness assessments and completes physical health assessments, verifying medications at admission and updating as needed.
  • Provides education and support regarding annual PEs, coordinates physical exams, routine medical and dental consultations, and accompanies persons served when needed.
  • Assists staff in meeting MAP standards and provides education and support.
  • Leads or co-leads groups in the area of Health & Wellness.
