About the position
The Registered Nurse (RN) position in the Hospital At Home program at UMass Memorial Health is a full-time role focused on delivering exceptional patient care in a home setting. The RN will utilize the nursing process to manage patient care from admission to discharge, ensuring that care is tailored to the developmental age of the patient population. This role involves patient assessment, care planning, and coordination of education for patients and their families, all while working collaboratively within a healthcare team.
Responsibilities
- Utilizes the Nursing Process for the delivery and management of patient's care from Admission to Discharge, appropriate to the developmental age of the patient population.
- Performs patient assessment/reassessment according to standard procedure.
- Develops and coordinates individual plan of care including discharge planning.
- Develops, implements/coordinates and documents Education Plan for patient/family/significant other.
- Directs, supervises, coordinates and evaluates nursing care within assigned clinical setting.
- Delegates appropriately to others in accordance with job description and competency level.
Requirements
- Current Massachusetts Nursing licensure and registration.
- Current unrestricted Driver's license.
- BLS certification is required for every RN unless an advanced certification is current (e.g., ACLS, PALS, NRP).
- Minimum 5 years of Nursing experience.
- Thorough knowledge of the discipline of Nursing.
Nice-to-haves
- Bachelor of Science degree in Nursing.
- Homecare or Telehealth experience.
- Phlebotomy, IV insertion, and telemetry experience.
