About the position

The Registered Nurse-Hospice is responsible for planning, coordinating, providing, and documenting care for terminally ill patients from admission to discharge. This role involves developing and implementing a hospice care plan tailored to each patient's needs while ensuring compliance with relevant standards and regulations.

Responsibilities

• Plan and coordinate care for terminally ill patients.
• Document patient care and progress accurately.
• Develop and implement individualized hospice care plans.
• Ensure compliance with accrediting organization standards and regulations.
• Utilize physicians' orders and professional nursing skills in patient care.

Requirements

• Graduate from an accredited nursing school required.
• Licensed as a Registered Nurse (RN) in the state of practice required.
• Valid driver's license and automobile insurance required for home visits.
• Basic Life Support (BLS) certification required.
• 1 year of acute nursing experience preferred.
• Prior experience in home health or hospice preferred.
• Bachelor's Degree in nursing preferred.
