About the position

As a BCN Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness, but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers. Responsibilities

  • Deliver compassionate and comprehensive health care services to Hospice patients in a home care setting.
  • Create and implement a holistic plan of care based on the patient’s goals and preferences.
  • Serve as the patient advocate to support, guide, and coordinate care for patients, families, and caregivers.
  • Manage symptoms through pharmacologic and non-pharmacologic interventions for optimal comfort, functionality, and safety.
  • Conduct frequent monitoring and follow-up to evaluate the efficacy of treatments and plans of care.
  • Provide emotional support to both the patient and the caregiver.
  • Work autonomously and within an interdisciplinary team.

Requirements

  • A graduate of an accredited school of registered nursing.
  • Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or participating COMPACT state Board of Nursing.
  • One year of general nursing experience is preferred; new nursing graduates will be considered.
  • Skilled in nursing practice and able to maintain professionalism in moments of emotional stress.
  • Provide care in a compassionate nature while serving a diverse population respectfully and professionally.
  • Complies with accepted professional and ethical standards and principles.
  • Understands and supports the hospice philosophy and principles of death and dying.
  • Possesses and maintains current CPR certification.
  • Have satisfactory references from nursing school, previous (or current) employers and/or professional peers.
  • Have excellent observation, good nursing judgment and communication skills.
  • Ability to provide accurate and timely documentation.
  • Self-directed to work with minimal supervision and within an interdisciplinary team.
  • Flexible and cooperative in fulfilling role obligation to meet the needs of patients and BCN.
  • Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements.
