Registered Nurse - Hospice Admissions - San Luis Obispo
About The Position
Central Coast Home Health and Hospice is seeking a Per Diem Hospice Admit Nurse to join their Hospice team in San Luis Obispo. The Admit Nurse is responsible for completing an initial assessment of the patient to determine hospice needs, counseling the patient and their family, and providing education as needed. This role requires accuracy, flexibility, and the ability to work independently. The nurse will deliver high-quality care to hospice patients in their homes, collaborating with an interdisciplinary team to promote patient and family choice, ensure continuity of care, and enhance the quality of life for dying patients. This position involves managing one's own caseload and work schedule.
Requirements
- Current California RN License
- At least one year of professional nursing experience required
- CPR certified (Healthcare Provider)
- Reliable vehicle to drive for work
- Current Driver's license and clean driving record
- Have and maintain auto insurance coverage
Nice To Haves
- Hospice &/or Home Health experience is a plus
- Understanding of the "hospice philosophy" and issues of death and dying
