About the position
The Registered Nurse-Home Health is responsible for developing and implementing personalized home health plans in collaboration with the Case Manager, ensuring compliance with relevant standards and regulations. This role involves planning, coordinating, and documenting care for a designated caseload of patients, while also participating in quality assessment and performance improvement activities.
Responsibilities
- Develop and implement home health plans based on physicians' orders and patient needs.
- Plan, coordinate, and document care for an assigned caseload of patients.
- Participate in Quality Assessment and Performance Improvement (QAPI) activities.
Requirements
- Graduate from an accredited and approved school of nursing accepted for licensure in the state of practice.
- Licensed as a Registered Nurse (RN) in the state of practice.
- Valid driver's license, automobile insurance, and own transportation for home visits.
- Basic Life Support (BLS) certification is required.
- 1 year of acute nursing experience is preferred.
Nice-to-haves
- Bachelor's Degree in nursing is preferred.
- Prior experience in home health or hospice is preferred.
