About the position

As a Home Health Registered Nurse at CenterWell Home Health, you will play a crucial role in providing personalized and comprehensive care to patients in their homes. This position involves managing patient care, coordinating with healthcare professionals, and ensuring that patients receive the necessary follow-up and support to achieve optimal health outcomes. You will work under the supervision of the Clinical Manager and collaborate closely with physicians to develop and implement effective care plans.

Responsibilities

  • Provide admission, case management, and follow-up skilled nursing visits for home health patients.
  • Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
  • Confer with physicians to develop the initial plan of treatment based on physician's orders and initial patient assessment.
  • Provide hands-on care, management, and evaluation of the care plan, teaching the patient in accordance with physician orders, under Clinical Manager's supervision.
  • Revise care plans in consultation with physicians based on ongoing assessments and as required by policy/regulation.
  • Coordinate appropriate care with various healthcare personnel, including Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
  • Report patient care, condition, and progress to the patient's physician and Clinical Manager continuously.
  • Implement patient care plans in conjunction with patients and families to assist them in achieving optimal resolution of needs/problems.
  • Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing written personal care instructions/care plans that reflect the current plan of care.
  • Monitor the appropriate completion of documentation by home health aides/personal care workers as part of supervisory responsibilities.
  • Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
  • Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.
  • Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimum patient care.

Requirements

  • Diploma, Associate or Bachelor Degree in Nursing
  • Minimum of one year nursing experience preferred
  • Strong med surg, ICU, ER, acute experience
  • Home Health experience a plus
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational and communication skills
  • Valid driver's license, auto insurance, and reliable transportation.
