Full-time - 40 hours/week Monday - Friday, 8:00am-4:30pm occasional on-call $38- $43/hour LIFE (Living Independence for the Elderly) is a Medicare/Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility. Responsibilities

  • Provides skilled nursing care and prescribed treatments to participants primarily in the day health center/clinic demonstrating quality care delivery by physicians' orders.
  • Directs and supervises the provision of quality para-professional clinical care to participants within the clinic/day center setting.
  • Participates as an on-call RN, including home visits, by the Albright LIFE on-call policies and procedures.
  • Conducts a comprehensive assessment of each new participant within the first week of program enrollment and conducts re-assessments and evaluations of each participant, per the LIFE standards.
  • Develops a comprehensive plan of care; provides routine nursing care for ongoing and episodic illnesses by the plan developed by the Interdisciplinary Team.
  • Maintains complete and accurate clinical documentation within established LIFE timeframes.
  • Manages the clinic schedule to ensure participants are seen by the RN and physician for initial, annual, periodic, and unscheduled assessments, acute issues, etc.
  • Establishes follow-up visits in the clinic as well as with specialty physicians and other providers.
  • Responsible for the supervision, orientation, evaluation, scheduling, and management oversight of the clinic LPNs and medication techs.
  • Other duties as assigned.

Requirements

  • RN or BSN Degree from a school of professional nursing; license and current registration to practice as a Registered Nurse in the Commonwealth of Pennsylvania.
  • One year of experience working with a frail or elderly population.
  • Ability to lift up to 25 pounds and to push wheeled equipment with weights up to 200 pounds throughout the center.
  • Must possess a valid PA driver’s license and be able to travel and work flexible hours as needed.
