About the position
The RN Case Manager at Help at Home is responsible for supporting clients and caregivers in the Atlanta, GA area by ensuring high-quality care and support solutions. This role involves monitoring personal care and skilled nursing services, coordinating clinical services, and conducting in-home assessments to determine client needs. The position aims to enable clients to maintain their independence and quality of life through effective management and oversight of care services.
Responsibilities
- Monitor the personal care, companionship, and skilled nursing services provided by direct care staff.
- Coordinate clinical services and ensure quality care services are being provided to clients.
- Perform in-home assessments of clients to determine their support and clinical needs.
- Conduct scheduled supervisory visits to clients' homes.
Requirements
- Current State of Georgia or multistate RN license
- Two (2) years of RN experience
- Current CPR certification
- Valid driver's license
- Professional written and verbal communication skills

- Home care experience is a plus, but not required
