About the position

The Registered Nurse Case Manager at Cabrillo Hospice is responsible for planning, organizing, and directing hospice care for patients and their families. This role emphasizes community health education and involves comprehensive assessments to determine hospice needs, developing care plans, and coordinating patient care. The position requires a strong focus on patient and family communication, ensuring continuity of care, and participating in quality improvement activities.

Responsibilities

  • Complete initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs.
  • Provide a complete physical assessment and history of current and previous illnesses.
  • Utilize all elements of the nursing process to provide professional nursing care.
  • Assess and evaluate patient's status by writing and initiating a plan of care.
  • Regularly re-evaluate patient and family/caregiver needs and revise the plan of care as necessary.
  • Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
  • Administer medications and treatments as prescribed by the physician.
  • Counsel the patient and family in meeting nursing and related needs.
  • Provide health care instructions to the patient as appropriate per assessment and plan.
  • Act as Case Manager and coordinate patient care for assigned caseload.
  • Maintain accurate and relevant clinical notes regarding patient's condition and care given.
  • Communicate with the physician regarding the patient's needs and report changes in condition.
  • Teach the patient and family/caregiver self-care techniques as appropriate.
  • Participate in on-call duties as defined by the on-call policy.
  • Supervise ancillary personnel and delegate responsibilities when required.

Requirements

  • Registered nurse with current licensure to practice professional nursing in California.
  • Graduate of National League for Nursing accredited school of nursing.
  • Current CPR certification.
  • Minimum of one year of experience as a professional nurse within the last three years or a baccalaureate degree in nursing from an accredited program.
  • Minimum of two years' experience, with at least one in public health, home care, or hospice nursing preferred.
  • Self-directed and able to work with minimal supervision.
  • Demonstrates excellent observation, problem solving, verbal and written communication skills.
