About the position

The RN/LPN position in the Primary Care department involves providing a variety of patient care services under the direction of the Clinical Nurse Supervisor. The Staff Nurse will be responsible for patient triage and treatment, ensuring patient comfort and wellbeing, and performing clerical duties as assigned. This role is part of the Department of Clinical Health Services located at 1575 Blue Hill Avenue, Mattapan, MA 02126.

Responsibilities

  • Provides direct patient care, utilizing the nursing process.
  • Assesses patients including vital signs, height, weight, BMI, allergies, medications and reason for visit.
  • Performs procedures such as the administration of medications either orally or by injection and immunizations as ordered, PPDs, suture removal, wound care, dressing changes, ear irrigation, EKG's, visual acuity screening, audiometry, O2 sats, blood glucose per Hemocue protocol, peak flow, nebulizer treatments, etc.
  • Collects specimens from patients for laboratory tests and notifies requesting providers of abnormal results.
  • Performs lab testing such as urine pregnancy test, rapid streps and routine urinalysis using correct protocols and procedures.
  • Triages patients by telephone or directly as walk-ins to identify urgency and level of care needed.
  • Makes appropriate referrals required for the management of patient care.
  • Assists providers with procedures such as suctioning, I&D, IUD insertion, endometrial biopsy, straight catheterization, Colposcopy.
  • Provides or arranges for patient education, such as instructing patients in the proper use of medical equipment, medication administration, diabetic teaching and other nursing care regimens.
  • Ensures that follow-up is made when patients miss scheduled appointments in order to determine reason and reschedule.
  • Assures appropriate scheduling and assists with patient flow.
  • Provides translation as needed.
  • Assists providers with calling patients regarding abnormal labs, sending medication refills and paperwork at the request of the provider.
  • Records in patient's medical record all care provided and ensures that appropriate forms are properly completed for billing purposes.
  • Monitors needs for equipment repair/replacement and notifies nursing supervisor.
  • Participates in individual or group teaching exercises.
  • Precepts nursing and medical assistant students as needed.
  • Provides follow-up on patients seen in the ER, hospital and patients with chronic diseases and abnormal labs.
  • Works on special programs/projects as assigned.
  • When assigned, works the telephone call center and is assigned to prescription drug line and call center per Nursing Supervisor.
  • Assigned to work with a NP or MD and assist with paperwork, medication refills or other tasks asked to perform.
  • Maintains current licensure through in-service education and obtaining CEUs.
  • Serves on committees as needed/assigned.
  • Performs other related duties as required or assigned.

Requirements

  • A graduate of an accredited School of Registered Nursing and current Commonwealth of Massachusetts Registration is required.
  • At least six months of experience as a Registered Nurse.
  • Ambulatory / outpatient care experience as a nurse, medical assistant or other clinical support position.
  • Current certification in CPR.
  • Excellent organizational skills, basic computer operations skills such as Word and Excel, as well as demonstrated oral and written communication skills.
