About the position

Under the general direction of nursing leadership and with provider collaboration and guidance, the Registered Nurse utilizes evidence-based practices in accordance with the State Nurse Practice Act and policies and procedures of Hartford Healthcare Hospital Outpatient Departments (HODs) and Medical Group, in a variety of clinical settings. This position facilitates care for a varied population of patients, which includes nursing standards of practice to meet the physical, emotional, spiritual, cultural, and educational needs of the patient and family.

Responsibilities

  • Provides nursing care within a non-acute setting in collaboration with the inter-disciplinary team and provider according to the patients physical, emotional, spiritual, cultural, and educational needs of the patient and family.
  • Coordinates and provides holistic patient centered care.
  • Utilizes evidence-based practice in accordance with the department and/or medical group policies and procedures to ensure safe quality care and customer service.
  • Incorporates art, advocacy, science and ethics in nursing effectively in their daily practice.
  • Demonstrates and actively supports research utilization through inquiring into evidence-based practices.
  • Evaluates and reviews the plan of care working with the interdisciplinary team to meet the diverse needs of the patient and family.
  • Identifies and reliably documents problems and expected outcomes based on a plan of care for a varied population of patients.
  • Maintains confidentiality of all patient information.
  • Assumes responsibility for self-improvement in collaboration with the leadership team by identifying, communicating and seeking resources or instruction/validation to meet own learning needs, and is self-directed in maintaining currency of validations.
  • Embodies the HHC leadership behaviors and company values and effectively utilizes the HHC operating model.
  • Completes required learning by designated deadline.
  • Maintains a clean, neat, and safe environment for patients and staff, ensuring supplies and equipment are stored in an organized and efficient manner.
  • Performs other related duties as required.

Requirements

  • Graduate of an accredited School of Nursing.
  • Associate's degree in nursing. Bachelors preferred.
  • Successful completion of nurse residency, if applicable.
  • Registered Nurse, State of CT.
  • Certified BLS Provider credentialed from the American Heart Association (AHA).
  • Additional certifications may be required in specific specialties.
  • Certification in a specialty a plus.

Nice-to-haves

  • Membership in a professional nursing organization is preferred.
  • Ability to speak another language in addition to English a plus.
