About the position
The position is responsible for coordination of care to meet identified needs of the patient/family. It involves supervising the management of patient care utilizing critical thinking and nursing processes, discharge planning, resource identification, linking and scheduling patients with required services, monitoring care delivery, advocating for the patient, care planning, and evaluating outcomes. The role coordinates patient care across the healthcare continuum and assists other team members with meeting individual patient needs. Flexibility of hours may be required.
Responsibilities

- Conduct discharge planning and resource identification.
- Link and schedule patients with required services.
- Monitor care delivery and advocate for the patient.
- Plan care and evaluate outcomes.
- Assist team members with meeting individual patient needs.
Requirements
- Bachelor’s degree (BSN) required; Associate Degree in Nursing (ADN) will be considered if enrolled in a BSN program with degree completion within 2 years.
- Minimum 1 year of current experience in acute care area of expertise; 2 years preferred.
- Trauma nursing experience preferred.
- Must challenge and successfully pass the Basic Dysrhythmia course curriculum within 21 days of hire.
- Basic computer skills/experience with computerized documentation systems required.
Nice-to-haves
- Current Certified Emergency Nurse (CEN) or related certification.
Benefits
- Compensation range of $49.31 - $73.35 per hour depending on qualifications and experience.
- Overtime pay for hours worked beyond 8 hours in a 12-hour shift.
