About The Position

The Registered Dietitian is an integral member of the interdisciplinary health professional team and is responsible for the nutrition care, disease management and education of patients. The Dietitian initiates a cost effective nutrition therapy comprised of patient assessment, the development and implementation of a patient-centered, goal-focused care plan, with ongoing evaluation. Nutrition care plans are outcome-focused, incorporate evidence based best practice, are formulated to optimize both clinical and fiscal outcomes, and emphasize health promotion and disease prevention. The Dietitian is responsible for the continuum of care of patients with effective, evidence based strategies to self manage disease thereby decreasing the risk associated complications and hospital admissions. As part of patient care, the Dietitian is responsible for educating patients/caregivers about their overall disease and especially how it relates to nutritional management. The Dietitian is part of and promotes a health care team approach to patient care. This position will provide support to collaborative family practice teams and chronic disease management teams located in the Pictou area, and is broken down as follows between the two clinics: 0.50 FTE at Pictou West Health Centre 0.50 FTE at One Door Chronic Disease Management Centre The Dietitian will provide health assessment, interventions and appropriate follow up in accordance with acts, regulations, policies, standards, guidelines, mission and objectives of the Registered Dietitians of Canada, the Nova Scotia Dietetic Association, the Department of Health and Wellness - Province of Nova Scotia and the Nova Scotia Health. Please note this position requires travel to facilities within Pictou County. A valid driver’s license and access to reliable transportation is required.

Requirements

  • Bachelor of Science in Human Nutrition at a recognized University required
  • Completion of Dietetic internship (a 1 year integrated or post graduate program) required
  • Licensure by Nova Scotia Regulator of Dietetics required
  • Certification with the Canadian Board of Certified Diabetes Educators (CDE) required (or willingness to obtain within 2 years)
  • Certification in insulin dose adjustment for adults required
  • Certification in insulin dose adjustment for pump therapies required
  • Minimum of two (2) years related experience required (i.e. Primary Health Care, diabetes & chronic illness, community-based services)
  • Access to reliable transportation for travel for work
  • A valid driver’s license

Nice To Haves

  • Competencies in other languages an asset; French preferred
