About the position

The position involves providing nutritional services to in-center and home patients to enhance their nutritional status and improve clinical outcomes. The role requires collaboration with an interdisciplinary healthcare team to support quality enhancement and performance improvement activities, ensuring that patients achieve their individualized goals as determined by their physicians.

Responsibilities

  • Assess patient's knowledge of diet and kidney disease and provide education based on their learning needs.
  • Provide detailed education to patients, families, and caregivers regarding nutritional status and related lab parameters.
  • Complete nutritional assessments and participate in comprehensive interdisciplinary assessments.
  • Develop individualized comprehensive care plans in collaboration with the interdisciplinary team.
  • Calculate diet prescriptions according to standard practices.
  • Document progress notes for chronic dialysis patients according to FMCNA guidelines.
  • Maintain accurate documentation in medical records as per FMCNA policies and regulations.
  • Conduct ongoing nutrition assessments and counseling to help patients achieve effective nutritional status.
  • Identify malnourished patients and collaborate with the team to implement appropriate interventions.
  • Review each patient's albumin level and body weight monthly.
  • Implement care plans with patient and family involvement.
  • Monitor adherence to nutrition therapy and adjust as needed, documenting any barriers to achieving goals.
  • Collaborate on Urea Kinetic Modeling recommendations to meet physician prescriptions.
  • Manage recommendations for mineral metabolism, anemia, and nutritional status with the interdisciplinary team.
  • Report on nutrition QAI results and participate in the QAI program.
  • Coordinate Pre-ESRD education with team members as appropriate.
  • Work with Case Manager to coordinate patient education and referrals for nutritional supplements.
  • Review nutrition-related lab results and counsel patients accordingly.
  • Communicate lab results and treatment recommendations to healthcare providers.
  • Participate in staff meetings and care plan meetings as applicable.
  • Consult with FMS Corporate Dietitian regarding nutrition concerns as needed.
  • Maintain and improve knowledge and skills for competent practice.
  • Ensure work areas are safe and clean.

Requirements

  • Registered Dietitian as per Commission on Dietetic Registration.
  • Board Certified Specialist in Renal Nutrition encouraged.
  • Current state licensure if applicable.
  • Minimum of 1-year professional work experience in clinical nutrition as a Registered Dietitian.
  • Demonstrated leadership competencies and skills, including teaching ability, strong organizational skills, and excellent communication.
  • Basic computer skills required.
