About the position

Form Health is seeking an experienced Registered Dietitian to provide individualized medical nutrition therapy to patients through a telehealth platform. The role focuses on supporting patients in their weight management journey, utilizing evidence-based practices and fostering a non-judgmental, empathetic relationship. The position is integral to enhancing the medical weight loss program in a dynamic environment.

Responsibilities

• Provide ongoing medical weight management support to patients via video visits.
• Establish effective, non-judgmental relationships with patients.
• Create customized nutrition plans based on individual patient needs.
• Provide nutrition education and counseling on healthy eating and behavior change.
• Review and give feedback on patients' daily food photos.
• Support patients through daily text messaging in the Form Health app.
• Lead patient support groups during some evenings as needed.
• Contribute to the growth and enhancement of the medical weight loss program.

Requirements

• Registered Dietitian with 3-5 years of experience in counseling and behavior modification techniques.
• Experience working with patients with obesity in a medical weight loss clinic.
• Strong cognitive-behavioral therapy counseling techniques.
• Certified Specialist in Obesity and Weight Management (CSOWM) highly preferred.

Nice-to-haves

• Board certification in obesity preferred.
• Experience with weight loss medications and meal replacement plans.
