About the position

The Registered Nurse for Hospice is responsible for planning, organizing, and directing hospice care, focusing on community health education and experience. This role involves assessing patient needs, developing care plans, and providing professional nursing care in patients' homes and communities. The nurse will work closely with patients and their families to ensure comfort and quality of life during end-of-life care.

Responsibilities

  • Completes initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs.
  • Provides professional nursing care utilizing all elements of the nursing process.
  • Assesses and evaluates patient's status by writing and initiating a plan of care, regularly re-evaluating needs, and revising the plan as necessary.
  • Uses health assessment data to determine nursing diagnosis and develops a care plan that incorporates palliative nursing actions.
  • Initiates appropriate preventive and rehabilitative nursing procedures, administering medications and treatments as prescribed.
  • Counsels the patient and family in meeting nursing and related needs and provides health care instructions as appropriate.
  • Assists the patient with activities of daily living and facilitates efforts toward self-sufficiency and comfort care.
  • Acts as Case Manager when assigned, coordinating patient care for the assigned caseload.
  • Maintains accurate clinical notes regarding patient's condition and care given, and communicates with physicians about patient needs.
  • Teaches self-care techniques to patients and families, providing medication and diet instructions as ordered.

Requirements

  • Registered nurse with current licensure to practice professional nursing in the state.
  • Maintains a current CPR certification.
  • Must have a valid driver's license and an insured automobile in good working order.
  • Minimum of one year of experience as a professional nurse within the last three years, or a baccalaureate degree in nursing and a current RN license.
  • Preferred: Minimum of two years' experience, with at least one in public health, home care, or hospice nursing.
  • Self-directed and able to work with minimal supervision.
  • Demonstrates excellent observation, problem-solving, verbal and written communication skills.

Nice-to-haves

  • Experience in hospice or palliative care nursing.
  • Management experience is not required but is a plus.
