About The Position

Transition Home Healthcare is seeking an experienced Quality Assurance Nurse (Home Health) to join their team. This role is responsible for ensuring the delivery of high-quality home health services by monitoring and evaluating clinical documentation and providing staff education. The position involves reviewing and monitoring clinical documentation, conducting audits, and ensuring compliance with regulatory standards and best practices in home health care. The role is OnSite in Houston, TX.

Requirements

• Valid Registered Nurse (RN) license or Licensed Vocational Nurse (LVN) in Texas.
• Minimum of 1 year of experience in a quality assurance role in home health, with at least 2 years of experience in home health care.
• Strong understanding of home health care regulations, quality assurance processes, industry best practices, OASIS, Value Based Purchasing & Star Ratings.
• Excellent analytical skills, attention to detail, and ability to identify and resolve issues.
• Strong communication and interpersonal skills.

Nice To Haves

• RN BSN preferred.
• OASIS certification is preferred.
