About the position

Abound Health is seeking a full-time Qualified Professional to provide support and services to clients in the Franklin, NC area. This role is pivotal in upholding the company's mission and values by delivering compassionate care and education to Service Recipients receiving Innovations Waiver or State Funded Services. The ideal candidate will be dedicated to making a positive impact in the lives of others and will work collaboratively with a team that values integrity and respect.

Responsibilities

  • Review assigned participant's service/grid notes in a timely manner
  • Review all verification/documentation of services for appropriate signatures and documentation per the NC DHHS Records Management and Documentation Manual
  • Ensure assigned direct care staff records are current
  • Ensure assigned service recipient clinical records are current
  • Conduct and document clinical and employee supervisory visits to ensure Medicaid and Abound's Policies/Procedures are followed
  • Conduct annual performance evaluations, direct care staff development plans, disciplinary meetings/documentation with direct care staff
  • Participate in required staff meetings
  • Participate and advocate for the recipient in the recipient's ISP meetings
  • Develop individualized short term goals, task analysis and interventions for assigned recipients
  • Complete recipient's annual skills and preference assessments (as applicable)
  • Ensure all RDSPs are completed (as applicable)
  • Ensure client specific training's are completed for every recipient/staff relationship
  • Enter goals into OnTarget
  • Complete TARs (as applicable) in Provider Direct before annual ISP
  • Review weekly billing to ensure hours are used or complete deviation of service forms
  • Complete quarterly progress reports
  • Review and update task analysis per recipient's goal frequency
  • Ensure staffing patterns are covered and back up staff is available
  • Resolve and document recipient/guardian grievances and report to Supervisor
  • Complete significant event and incident reports within 24 hours (recipient and employee/contractor)
  • Schedule direct care staff/contractors to provide direct care, ensuring compliance with service authorizations and waiver limitations
  • Assist the recipient and families with finding and procuring appropriate resources and services in the community
  • Represent Abound Health at Community Events
  • Provide privacy and confidentiality of all participants' records
  • Assure participants are free from abuse, mistreatment, and/or neglect and report any incidents of such to supervisor
  • Treat each participant with dignity and respect
  • Develop and demonstrate a thorough understanding of Abound's philosophy and purpose
  • Provide a positive atmosphere which facilitates growth and learning of coping skills.

Requirements

  • Must be at least 18 years of age
  • Valid Driver License
  • Safe Driving Record
  • Criminal, LEIE, NC Health Care Registry Check and Sexual Offender Background checks clear of health and safety risks
  • Reliable and safe transportation
  • Current Auto Insurance and Car Registration
  • Graduate of a college/university with a Master's degree in a related human service field and has 1 year of full-time IDD experience accumulated, or
  • Graduate of a college/university with a baccalaureate degree in a related human service field and has 2 years of full-time IDD experience accumulated, or
  • Graduate of a college/university with a baccalaureate degree in an unrelated human service field and has 4 years of full-time IDD experience accumulated
