Responsibilities

• Provide physical therapy services to patients in a Skilled Nursing Facility (SNF) setting.
• Develop and implement individualized treatment plans based on patient assessments.
• Collaborate with healthcare team members to ensure comprehensive patient care.
• Document patient progress and modify treatment plans as necessary.
• Educate patients and their families on treatment plans and home exercises.

Requirements

• Active NC SNF UNIT license.
• Minimum of 2 years of experience as a Physical Therapist.
