About The Position

Using a one to one and group person-centered counseling model, Community Oriented Recovery and Empowerment (CORE) program seeks to improve the functioning of individuals living with mental health diagnoses, histories of chemical and alcohol dependence, or physical disabilities, helping them achieve personal goals in the areas of recovery, employment or independent functioning. Under the supervision of the Program Director, the CORE Psychosocial Rehabilitation Specialist will provide CORE services to HARP eligible clients primarily for Psychosocial Rehabilitation (PSR). The CORE Counselor may also provide additional services such as CPST and empowerment services, as needed based on the candidates licensing and/or credentialing. Psychosocial Rehabilitation (PSR) is designed to assist an individual in improving their functional abilities to the greatest degree possible in settings where they live, work, learn, and socialize. Rehabilitation counseling, skill building, and psychoeducational interventions provided through PSR are used to support attainment of person-centered recovery goals and valued life roles. CORE services are intended to contribute to the reduction of unnecessary hospitalization/ institutional level of care and improved societal integration of those served. Service delivery is designed to be flexible and individualized and is focused on the promotion of skill development and identification of community supports in order to overcome deficits related to function and motivation. Psychosocial Rehabilitation is provided individually and in groups.

Requirements

  • Bachelor’s degree
  • Social work background or relevant experiences in supporting individuals pursuing recovery
  • Spanish language fluency

Responsibilities

  • Engaging the individual in shared decision making around service and recovery planning.
  • Regular assessment of progress toward goals and updating the individual service plan.
  • Communicating with collaterals, including other providers, to ensure integrated, collaborative services and access.
  • Interventions designed to develop or improve personal coping strategies, prevent crises, and promote recovery.
  • Provide Mental health psychoeducation
  • Coping skills and stress reduction/management skills training
  • Problem-solving skills training
  • Medication education and self-management skills training and Crisis prevention planning
  • Individualized, goal-driven interventions that facilitate, promote, and improve functioning in living, working, learning, and social environments.
  • Identification of strengths and resources which may aid the individual in achieving recovery
  • Resource and support coordination to help an individual identify, assess, develop and secure available supports necessary for success towards their rehabilitation goals.
  • Identification of personal, environmental, and behavioral health barriers that may impede the development of skills necessary for functioning in work or school, with peers and with family and friends.
  • Develop, strengthen, and support the individual's independent community living skills and community participation through self-advocacy and system navigation; promoting access to necessary rehabilitative, medical, social, academic, and other services and supports.
  • Other tasks outlined as stated in the NYS CORE operations Manual
  • Based on a new hires credentialling or licensing. Opportunities to provide other CORE services such as Empowerment Peer Services or CPST Services may be available
