About the position

The Program Manager for the Structured Family Caregiving (SFC) program at Family Support Services of Indiana LLC is responsible for overseeing the operations, development, and management of the SFC program. This role focuses on providing support to family caregivers who assist individuals with complex healthcare needs, ensuring they have the necessary resources, training, and support to deliver high-quality care while complying with state regulations and policies.

Responsibilities

  • Oversee the development, implementation, and management of the Structured Family Caregiving program.
  • Manage program goals and outcomes, ensuring adherence to the mission of providing family caregivers with the support and resources needed to care for individuals in the home and community setting.
  • Ensure that services are delivered in compliance with federal and state regulations, including Medicaid guidelines.
  • Provide leadership in recruitment, onboarding, and retention of family caregivers.
  • Offer resources, training, and support to caregivers to help them succeed in their roles.
  • Monitor caregiver satisfaction and address concerns in a timely manner.
  • Monitor and evaluate the program's performance, ensuring compliance with state and federal regulations.
  • Conduct regular audits to ensure quality of care and adherence to Medicaid guidelines.
  • Serve as the primary point of contact for the Structured Family Caregiving program, fostering relationships with caregivers and stakeholders.
  • Collaborate with the Division of Aging, Medicaid, and other health services agencies to enhance program delivery.
  • Advocate for the needs of caregivers within the state healthcare system.
  • Develop and implement caregiver training programs focused on enhancing caregiving skills.
  • Develop and manage the budget for the program, ensuring efficient resource allocation.
  • Maintain accurate records related to caregivers, clients, and service outcomes.

Requirements

  • Bilingual in Spanish and English.
  • 2 years of experience in Structured Family Caregiver (SFC) Program management.
  • Strong organizational skills and ability to manage multiple priorities.
  • Excellent communication and interpersonal skills.
  • Ability to analyze data and prepare reports.
  • In-depth understanding of healthcare regulations, Medicaid, and aging services.
  • Driver's License is required.

  • Knowledge of Indiana's healthcare system and aging services.
  • Experience with Aged and Disabled Waiver Services.
  • Experience with Bureau of Disabilities Services (BDS) waiver.
