About the position

The Community Health Coordinator (CHC) at El Rio Community Health Center serves as a liaison and advocate for patients, collaborating with various health and educational organizations to address unmet healthcare needs. The role involves coordinating health promotion activities, developing educational materials, and facilitating community outreach to ensure comprehensive and accessible healthcare for all patients. The CHC is responsible for overseeing grant-funded projects, conducting health education sessions, and maintaining compliance with healthcare regulations.

Responsibilities

  • Coordinate and oversee activities of Ventanilla De Salud (VDS), Health Builders (HB), and other grant projects to meet program goals.
  • Consult on and oversee health promotion and education activities for individuals and groups.
  • Research and coordinate program activities with other departments and organizations.
  • Develop educational and marketing materials for program events and training sessions.
  • Provide outreach and presentations to community agencies and groups in need of healthcare access.
  • Support program projects and complete administrative duties within established time frames.
  • Participate in the development of educational materials for clients and community members.
  • Identify health concerns in clinic populations and provide basic health education.
  • Teach health promotion and disease prevention classes for target populations.
  • Facilitate training sessions and events sponsored by the program.
  • Coordinate logistics for community events and clinic activities.
  • Maintain reports for grantors and attend monthly meetings to present information.
  • Support evaluation and monitoring of program activities and assist in grant compliance.
  • Participate in recruitment and training of staff and volunteers for program duties.
  • Maintain a calendar of health center programs and outreach events.

Requirements

  • Bachelor's Degree in Health Care, Social Services, or related field from an accredited institution.
  • Three years of experience coordinating community outreach education and events.
  • Three years of experience as a Community Health Advisor or in an advocacy role.

Nice-to-haves

  • Master's Degree in Health Care, Social Services, or related field.
  • Community Health Advisor Certification from an accredited institution.
  • Three years of experience with governmental and private healthcare programs.
  • Experience writing grant proposals.
  • Advanced fluency in English and Spanish.
