About the position

The Primary Care Nurse position at Mercy Clinic Oakville is a newly created role designed to enhance patient care by managing triage calls and messages in collaboration with healthcare providers. This role offers a hybrid work option after an initial three-month on-site training period, with the expectation of attending monthly clinic meetings and occasional on-site visits. The position is structured to provide a supportive environment for both LPNs and RNs, focusing on professional growth and patient-centered care.

Responsibilities

  • Manage triage calls and messages for the clinic.
  • Collaborate closely with healthcare providers to ensure quality patient care.
  • Complete three months of on-site training before transitioning to a hybrid work-from-home schedule.
  • Attend monthly clinic meetings and be available on-site 1-2 times per week.

Requirements

  • Active LPN or RN license required.
  • For LPN: Diploma in Practical Nursing and 2-3 years of experience preferred.
  • For RN: Associate of Applied Science in Registered Nursing or Bachelor's degree in nursing (BSN) or 4 years of commensurate experience in lieu of the bachelor's degree, with 2-3 years of experience preferred.

Nice-to-haves

  • Experience in a clinical setting preferred.
  • Strong communication skills.
